While we acknowledge that multicentre studies offer advantages in terms of generalisability, our single-centre prospective design provides important strengths. The PETRA study referenced by the authors was conducted over a two-year period across multiple sites. This study, conducted at Kalafong Provincial Tertiary Hospital over 12 months, enrolled 248 patients from a well-defined catchment area serving the western region of Tshwane. This single-centre approach ensured standardised management

Conclusion

This study makes a valuable contribution to the preeclampsia literature by demonstrating that proteinuria severity, measured by the practical and accessible UPCR, correlates with earlier delivery and development of severe features, particularly HELLP syndrome. These findings have immediate clinical utility for risk stratification and patient counseling in settings like ours.

We agree with the authors that preeclampsia warrants continued research attention. Our work provides specific, actionable
